Nutrition Consultation Agreement
By booking and paying for your consultation, you agree to the following terms with Jess Wharton Nutrition (JWN).
1. General Terms
- Your initial consultation lasts 90 minutes. Follow-up sessions are 45 minutes. Longer or more frequent sessions can be arranged if you require additional support. Sessions will end at the scheduled time, even if you are late.
- In your first session, we will discuss and agree on the support you need. You may choose to end or continue consultations on an ongoing basis.
2. Payment
- All sessions must be paid for before they start. You will receive an invoice when booking your initial consultation.
- Missed or cancelled sessions with less than 48 hours’ notice must still be paid in full.
3. Cancellation
- Regular attendance is important for progress.
- Cancellations made within 24 hours of your session will incur a 50% late cancellation fee.
- Missing a session without prior discussion is considered a termination of your contract.
4. Refunds
- For program or package deals, cancelling follow-up appointments after the first session will result in a 50% refund.
- Any unused follow-up appointments 6 months after your initial session are non-refundable.
- If you cancel an initial consultation with less than 48 hours’ notice, no refund will be issued.
5. Confidentiality
- All consultations are confidential.
- Confidentiality may be lifted if there are serious safety concerns for you or others.
6. Complaints
- If you have any issues with the service, please provide written details to JWN.
7. Online & Phone Consultations
- You are responsible for a working internet connection for online sessions.
- If Jess Wharton is ready but you cannot connect, the session fee still applies.
- Phone sessions will be conducted on New Zealand landline or mobile numbers.
8. Nutritional Therapy
- Nutritional therapy identifies potential nutritional imbalances and works to support overall health.
- Nutritional therapy is complementary, not a substitute for medical care.
- Nutritional therapists do not diagnose or treat medical conditions.
- Clients must inform their GP or other healthcare providers about any nutritional strategies, supplements, or therapies provided by JWN.
9. Client Responsibilities
- Contact your GP about any health concerns.
- Inform your nutritional therapist about any medical diagnoses, medications, herbs, or supplements.
- Seek clarification from your therapist if you are unsure about any nutritional program or supplement dosing.
- Do not continue a supplement program beyond the agreed period without consulting your therapist.
- Recording consultations is not allowed without written permission from both JWN and the client.
